NHS (NDPP) 200-Page Diabetes Tracker Journal

Overview

The NHS NDPP needed a comprehensive 200-page journal to help patients track their progress, record lifestyle changes, and stay engaged with their diabetes prevention journey. The existing materials lacked structure and ease of use, making it difficult for patients to maintain tracking consistency. This work was completed in-house in Thrive Tribe, one of the providers for the NHS Diabetes Prevention Programme (NDPP).

Process

To create an intuitive and effective journal, I:

• Researched existing NHS materials and competitor health journals to identify best practices.
• Designed a structured layout system that was easy to navigate, with dedicated sections for goal setting, meal tracking, progress notes, and reflections.
• Developed a clean, NHS-compliant aesthetic using their brand colours, typography, and iconography for consistency.
• Used Illustrator & Photoshop to enhance visuals, creating custom icons and infographics to improve engagement.
• ​​​​​​​Conducted multiple revisions based on NHS feedback to refine usability and clarity.

Outcome

The final design was a professionally structured, 200-page journal that empowered users to:

• Easily track their progress with structured daily and weekly check-ins.
• Stay motivated with goal-setting prompts and guided reflection sections.
• Quickly reference educational resources with clear typography and infographics.
• Engage with the journal consistently thanks to its visually appealing and organised layout.
